Still Putting Up With Your Back Pain? Here's Why It Keeps Coming Back
Lower back pain is one of the most common reasons Australians visit a physiotherapist, and one of the most frustrating to live with. It slows you down, disrupts your sleep, and has an infuriating habit of returning just when you thought it was gone. If that sounds familiar, this article is for you. We explain exactly what is driving your lower back pain, why it keeps coming back, and how our physio team at PHYZFIT Health and Rehab can help support your recovery.

Why Lower Back Pain Is So Common and Why It Keeps Coming Back
The lumbar spine is one of the most load bearing structures in the body. Every time you sit, stand, lift or twist it absorbs and distributes force. Over time, repetitive loading patterns, muscle imbalances, prolonged poor posture and a lack of core stability create the conditions for pain to develop.
The reason lower back pain so often becomes persistent is that people manage the symptom without finding the cause. Rest, heat and anti inflammatories can take the edge off temporarily, but without addressing the specific movement fault, the weak muscle, or the lifestyle habit driving the problem, the pain reliably returns. A thorough physiotherapy assessment identifies exactly what is contributing so treatment can be targeted and lasting.
Common Symptoms
- Dull, aching lower back pain that is worse after sitting or first thing in the morning
- Stiffness that takes time to loosen up after periods of rest or inactivity
- Sharp catching pain with certain movements such as bending, lifting or twisting
- Pain that spreads into the buttocks, hip or down one leg
- Pins and needles, numbness or weakness radiating into the leg or foot
- Muscle spasm and tightness across the lower back and gluteal region
Common Causes
- Prolonged sitting and sustained desk or driving posture throughout the day
- Muscle and ligament strain from lifting, twisting or a sudden awkward movement
- Lumbar disc bulge or herniation pressing on nerve roots (commonly causing sciatica)
- Facet joint irritation or degeneration from repetitive loading
- Weak or poorly activating deep core stabiliser muscles
- Low activity levels, poor sleep posture, high stress and excess load on the spine
When Lower Back Pain Becomes a Nerve Problem
If your lower back pain is accompanied by pain, tingling or numbness travelling into your leg, there may be nerve involvement. This is commonly called sciatica and occurs when a lumbar disc bulge or tight piriformis muscle irritates the sciatic nerve as it exits the spine or travels through the hip region.
Sciatica can range from a mild ache in the buttock to severe burning pain that travels all the way to the foot. It rarely settles on its own without physiotherapy and without appropriate management can worsen over time. A thorough assessment will identify whether nerve involvement is present and determine the most effective treatment approach for your situation.
How We Treat Lower Back Pain at PHYZFIT
Our physiotherapy team at PHYZFIT takes an evidence based, whole body approach to lower back pain. We do not simply treat where it hurts. We assess how you move, identify what is overloading the spine, and build a treatment plan that addresses the real cause so your recovery holds long term.
Thorough Assessment of the Lumbar Spine
We assess the joints, discs, muscles and neural structures contributing to your pain as well as your movement patterns and daily habits. You leave your first session knowing exactly what is wrong and what the plan is to address it.
Manual Therapy and Joint Mobilisation
Hands on treatment to the stiff or restricted joints of the lumbar spine and pelvis can significantly reduce pain and restore movement quickly. Joint mobilisation reduces muscle guarding, improves range of motion and interrupts the pain cycle keeping the back locked up. Many patients feel meaningful relief after their very first treatment session.
Dry Needling and Soft Tissue Therapy
Tight muscles in the lower back, glutes, hip flexors and piriformis are extremely common contributors to lower back symptoms. Dry needling and deep tissue release deactivate trigger points, reduce protective muscle tension and restore normal tissue quality so the spine can move freely.
Core Strengthening and Rehabilitation
Weakness in the deep core stabiliser muscles is one of the most consistent findings in people with recurrent lower back pain. A structured and progressive rehabilitation program targeting these muscles is the most important part of long term recovery. Without it the back remains vulnerable regardless of how much passive treatment is applied.
Education and Self Management
Understanding your condition and knowing what to do on the difficult days is just as important as what happens in the clinic. We make sure you leave every session with the knowledge and practical tools to manage your back, stay on track and prevent recurrence over the long term.
What to Expect at Your First Appointment
Your first session at PHYZFIT is a full assessment. We listen, we examine and we explain. You leave with a diagnosis, a clear treatment plan and a realistic understanding of what recovery looks like for you.
Book online or call us. Same day appointments available at Bexley North and Marrickville. No referral needed to get started.
We assess your joints, discs, muscles and movement patterns to identify what is actually driving your pain, not just where it is.
We do not just assess and send you home. Treatment begins at your first session so you feel a difference straight away.
You leave with a home program, a clear timeline and the tools to stop this coming back. We build strength and resilience, not dependency.
What You Should Not Do
- Do not rest completely for more than 2 days as prolonged rest slows recovery and increases stiffness
- Do not assume lower back pain will resolve on its own if it has been there for more than 2 weeks
- Do not rely only on pain medication and heat without addressing the underlying cause
- Do not continue with heavy lifting or aggravating activities without guidance on technique and load management
- Do not ignore leg symptoms such as tingling, numbness or weakness as these need prompt assessment
- Do not accept recurring lower back pain as normal as it responds very well to the right physiotherapy program
Seek urgent medical attention if you experience lower back pain alongside loss of bladder or bowel control, numbness in the groin or inner thighs, weakness in both legs, or severe pain following a fall or accident. These symptoms require immediate medical assessment.

How long does lower back pain take to resolve with physiotherapy?
Most people with acute lower back pain see significant improvement within 4 to 6 sessions. Chronic or more complex presentations such as disc injuries or recurring episodes typically require 8 to 12 sessions alongside a structured home exercise program. The sooner you begin targeted treatment the faster and more complete your recovery tends to be.
Can physiotherapy help with sciatica?
Yes. Physiotherapy is one of the most effective approaches for managing sciatica. Treatment addresses both the source of nerve irritation and the structures contributing to it. Most patients experience a meaningful reduction in leg symptoms within a few sessions once the right approach is applied.
Do I need a scan before seeing a physiotherapist?
No. In the vast majority of cases a thorough clinical assessment provides all the information needed to begin treatment effectively. Scans can be helpful for complex presentations but are rarely the first step and should not delay you from starting physiotherapy.
Do I need a referral to see a PHYZFIT physiotherapist?
No referral is needed to book directly with us. We accept private health insurance, Medicare Enhanced Primary Care plans, NDIS, WorkCover, SIRA, CTP and DVA. If you are unsure about your cover call us before your appointment and we will help you sort it out.
Is it safe to exercise with lower back pain?
In most cases yes, with appropriate guidance. Complete rest is rarely the right answer and often delays recovery. Your physiotherapist will advise you on which activities are safe, what to modify and how to progressively build back up. Movement done correctly is a core part of recovery, not something to avoid.
