Medicare CDM — How It Works
The Chronic Disease Management program allows eligible patients with chronic conditions to access up to 5 subsidised allied health sessions per calendar year.
See your GP
Speak with your GP about eligibility for a Chronic Disease Management plan. You must have a chronic condition that has been present for at least 6 months or is expected to last at least 6 months.
Get a GP Management Plan and Team Care Arrangement
Your GP prepares a GPMP and TCA which includes a referral to physiotherapy. You are entitled to up to 5 allied health sessions per calendar year under this plan.

DVA Physiotherapy — How It Works
PHYZFIT is registered with the Department of Veterans Affairs and welcomes DVA Gold and White card holders. We bulk bill DVA patients meaning there is no out of pocket cost for eligible cardholders.

DVA Gold card holders are eligible for all medically necessary physiotherapy with no out of pocket cost. DVA White card holders are eligible for treatment of accepted conditions. Call us to confirm your specific entitlements.
Get a referral from your GP
For most DVA physiotherapy services you need a referral from your GP or treating specialist. We encourage open communication with your GP throughout your treatment.

How many Medicare sessions can I use at PHYZFIT?
You can use all or part of your 5 annual CDM sessions at PHYZFIT. If you have already used some sessions with other providers your remaining sessions will be fewer. Your GP manages the total across all providers.
Does DVA cover all physiotherapy treatments?
