Why Shoulder Pain Is So Common and Why It Rarely Fixes Itself

The shoulder is the most mobile joint in the human body, which is exactly what makes it so vulnerable. It relies on a complex interplay of muscles, tendons, ligaments and joint structures to stay stable while allowing an enormous range of movement. Why Most Shoulder Pain Is Not Just a Shoulder Problem

One of the most important things our physiotherapists assess in shoulder pain is the role of the neck and upper back. Stiffness in the cervical spine can directly refer pain and nerve symptoms into the shoulder, upper arm and hand. What You Should Not Do

  • Do not push through shoulder pain assuming it will resolve without treatment if it has been there for more than 2 weeks
  • Do not rely on heat and anti inflammatories as your only strategy without identifying the underlying cause
  • Do not continue training through pain at the gym without guidance on load management and technique
  • Do not assume night pain is normal or that sleeping on a different pillow will fix it
  • Do not ignore arm or hand symptoms including tingling, numbness or weakness as these need prompt assessment
  • Do not accept a progressively stiffening shoulder without assessment as frozen shoulder is far easier to treat in its early stages

Seek urgent medical attention if you experience sudden severe shoulder pain following a fall or dislocation, complete inability to lift the arm, significant bruising or deformity, or chest pain that radiates into the shoulder. These symptoms require immediate medical assessment.

How long does shoulder pain take to resolve with physiotherapy?

Most people with acute shoulder pain see significant improvement within 4 to 6 sessions. More complex presentations such as rotator cuff tears, frozen shoulder or shoulder impingement typically require 8 to 12 sessions alongside a structured home exercise program. Starting the right treatment early gives you the best chance of a full and lasting recovery.

Can physiotherapy help with a rotator cuff tear?

Yes. The majority of rotator cuff tears, including partial thickness tears, respond very well to physiotherapy without the need for surgery. A targeted rehabilitation program focusing on the surrounding muscles can significantly reduce pain and restore full function. Your physiotherapist will advise whether imaging or specialist review is needed based on your assessment findings.

Can physiotherapy help with frozen shoulder?

Yes, and the earlier it is treated the better. Physiotherapy in the early stages of frozen shoulder can significantly reduce the severity and duration of symptoms. Manual therapy, specific stretching and guided exercise are effective in all stages of the condition. Left untreated, frozen shoulder can take 2 to 3 years to fully resolve on its own.
