Short Answer

In Sydney, a private physiotherapy consult typically costs $110 to $180 out of pocket. Medicare covers 5 sessions per year under a Chronic Disease GP Management Plan with a current rebate of $63.40 per session, refunded to your bank account within about 24 hours. Most private health extras cover $40 to $85 back per session on the day through HICAPS. At PHYZFIT Bexley North and Marrickville, fees start from a similar range and health fund rebates are processed on the spot.

Below we walk you through initial versus follow up fees, Medicare and health fund cover, workers comp and CTP options, and what actually makes a good physio worth the fee.

What is the average cost of physiotherapy in Sydney?

Standard private physiotherapy in Sydney sits between $110 and $180 for a 30 to 45 minute session. Initial consults are usually $10 to $30 higher than follow up appointments because more time is spent on assessment and treatment planning. Prices vary based on session length, clinic overheads, seniority of the physio and whether extras like dry needling, shockwave therapy or real time ultrasound are included.

Longer sessions of 60 minutes for complex spinal, neurological or post surgical rehab typically cost $180 to $250. Home visits and after hours appointments are usually charged at a premium of about $40 to $80 on top of the standard fee.

Typical Sydney physio fee ranges

  • Standard initial physio consult (45 min): $130 to $200
  • Standard follow up physio consult (30 min): $110 to $170
  • Extended or complex consult (60 min): $180 to $250
  • Sports physio, spinal rehab or post surgical rehab: $150 to $220
  • Dry needling and shockwave add ons: often included, sometimes $10 to $30 extra
  • Home visit or after hours: add $40 to $80

Can I claim physiotherapy on Medicare?

Yes. Medicare covers up to 5 physiotherapy sessions per calendar year under a Chronic Disease GP Management Plan (previously called an EPC or Enhanced Primary Care plan). Your GP writes the referral, sets the treatment goals and sends it directly to the physio.

The current Medicare rebate is $63.40 per session, and it is typically refunded to your bank account within about 24 hours of your appointment. Most Sydney physio clinics still charge a gap fee on top of the rebate because the Medicare rate does not cover the true cost of a proper session. At PHYZFIT we lodge your Medicare claim on the day, so the rebate lands in your account fast and you only carry the gap.

To qualify you generally need a chronic or long term condition such as ongoing lower back pain, arthritis, post stroke rehab, chronic knee or hip issues, or persistent musculoskeletal problems that need coordinated care from more than one health professional.

Does private health insurance cover physiotherapy?

Yes if you hold Extras cover (also called Ancillary cover). Physiotherapy sits under the Extras policy, not Hospital cover. The amount you get back depends on your fund, your tier of cover and how many sessions you have already used in the calendar year.

Typical rebates in Sydney sit at $40 to $85 per session. Higher tier policies from Bupa, HCF, Medibank, NIB, HBF, AHM and Australian Unity often rebate closer to the top of that range for the first several visits, then drop or cap out later in the year.

Most Sydney physio clinics run HICAPS, which lets you swipe your health fund card at the end of the appointment and only pay the gap. PHYZFIT Bexley North and Marrickville both use HICAPS so you never wait weeks for a paper claim.

What if my injury is from work, a car accident, DVA or NDIS?

In most of these cases you pay nothing out of pocket. The scheme pays the clinic directly at set fee rates.

  • Workers Compensation (icare NSW): Once your claim is approved and your treating doctor lodges an Allied Health Treatment Request, sessions are fully funded at the SIRA fee. No gap.
  • CTP Motor Accident (SIRA NSW): Same principle. Your CTP insurer approves the Allied Health Treatment Request and pays direct. No gap.
  • DVA White or Gold Card: Fully covered once your GP writes a D904 referral. No gap.
  • NDIS (plan managed or self managed only): Sessions are covered under Capacity Building Improved Daily Living. At PHYZFIT we accept plan managed and self managed participants. See our separate NDIS guide for more detail.

Why do physio prices vary so much between clinics?

Two clinics next door to each other can charge very different fees, and the difference is not random. These are the real drivers:

  • Time with the physio. A $90 clinic often runs 15 to 20 minute sessions with a lot of machine based treatment and minimal hands on work. A $150 clinic usually runs 30 to 45 minutes one on one with your physio the whole time.
  • Seniority and clinical focus. Sports physios, spinal specialists, women's health physios and titled physios have completed extra postgraduate training and generally charge more.
  • Assessment depth. A quality initial consult includes a full history, movement assessment, orthopaedic testing, treatment and a written home program. Rushed consults skip most of this.
  • Rehab progression. Good clinics track your progress, adjust your program, and give you clear timelines. Cheap sessions often become endless "come back next week" appointments.

If a physio is genuinely helping you recover, you should be doing fewer sessions over time, not more. That is often where the real value sits.

How many sessions of physio will I need?

Every case is different, but honest averages help you budget realistically:

  • Acute injury (sprain, strain, minor tear): 3 to 6 sessions over 4 to 8 weeks
  • Recurring or chronic pain (lower back, neck, shoulder): 6 to 12 sessions over 8 to 16 weeks
  • Post surgical rehab (ACL, rotator cuff repair, joint replacement): 12 to 24 sessions over 3 to 9 months
  • Sports and strength based rehab: often blends with strength and conditioning across a 12 to 20 week block

Ask your physio at the end of the first visit for a rough timeline. A confident, experienced physio should be able to give you a range.

⚠ Watch out for these red flags in a physio quote

  • Being told you need 20 plus sessions before the physio has even assessed you
  • Being locked into a prepaid package on your first visit
  • Every session ends with "come back in 3 days" with no progression or milestones
  • Machine only treatment with almost no hands on assessment or exercise progression

Frequently asked questions about physio costs

For most musculoskeletal problems, a properly qualified physio can help you avoid unnecessary scans, injections and surgery, and get back to activity faster than waiting it out. The value sits in the accuracy of the diagnosis and the quality of the rehab plan, not just the treatment on the day.

No. Physiotherapy is a primary contact profession in Australia. You can book directly without a referral. You only need a referral if you want to use a Medicare Chronic Disease Management plan, a DVA card, workers comp or CTP.

Not for the same session. You can use Medicare for some sessions and private health for others, but you cannot double claim a single appointment.

Bulk billed private physio is rare in Sydney because the Medicare rebate does not cover the true cost of a proper session. Some very high volume clinics offer bulk billing but tend to run much shorter appointments. PHYZFIT charges a gap on Medicare sessions and gives you a full 30 to 45 minutes one on one.

Not at PHYZFIT. The fee you are quoted includes the full session, any dry needling or shockwave used on the day, and your written home program. We will always tell you before adding anything that costs extra.

Speak up. A good physio will help you space out visits, build a strong home program, and use lower cost options like group strength based sessions where appropriate. Skipping care usually costs more in the long run.
