Short answer: Use ice for a fresh injury with swelling like a rolled ankle, sprained wrist or acute strain, ideally in the first 48 to 72 hours. Should you reach for the ice pack or the hot water bottle?

It is one of the most common questions we get at PHYZFIT. Grab the wrong one and swelling sits around for longer, stiffness lingers, and pain drags out. Grab the right one and you settle a fresh injury faster, loosen chronic tightness sooner, and give your recovery a real head start.

The Simple Rule

When should you use ice?

Use ice (cold therapy) when an injury is fresh, swollen or painful. That usually means the first 48 to 72 hours after a rolled ankle, sprained wrist, acute muscle strain, corked thigh or knock in sport.

Ice slows blood flow, dampens pain signals and can reduce the amount of swelling that builds up early on.

Use an ice pack wrapped in a tea towel. Apply for 15 to 20 minutes at a time every 2 hours in the first day or two. Never put ice directly on skin.

The Other Rule

When should you use heat?

Use heat (thermal therapy) when you are dealing with chronic muscle tightness, stiff joints, older aches or persistent pain that no longer has active swelling.

  • Long standing lower back pain or neck stiffness
  • Old chronic hamstring, calf or trap tightness
  • Warming up before exercise if you feel stiff
  • Period pain and tension type headaches
  • Sub acute injuries after the first 3 to 5 days when swelling has settled

Heat improves blood flow, relaxes muscles and can settle chronic pain signalling. Wheat packs, heat packs or warm showers all work. Apply for 15 to 20 minutes at a time.

What about the RICE, POLICE and PEACE and LOVE protocols?

Short answer: Physio guidelines have moved on from RICE (Rest, Ice, Compression, Elevation) to POLICE (Protection, Optimal Loading, Ice, Compression, Elevation) and more recently to PEACE and LOVE for soft tissue injuries.

  • Protection (avoid aggravating movement for 1 to 3 days)
  • Elevation
  • Avoid anti inflammatories and ice on chronic tendon injuries
  • Compression
  • Education about the injury
  • Load (gradually reload the tissue)
  • Optimism
  • Vascularisation (gentle cardio to promote healing)
  • Exercise (targeted rehab)

The take home is that early controlled movement and loading now sit alongside ice, not just rest and ice.

Common Mistakes

Can heat or ice make an injury worse?

Yes, if you use them at the wrong time. These are the most common mistakes we see in clinic.

  • Heat on a fresh sprain. Heat can increase swelling and bleeding in the first 48 hours. Stick to ice.
  • Ice directly on skin. Always wrap in a thin towel to avoid ice burns.
  • Ice for chronic tendon pain. Long term ice may slow tendon healing in tennis elbow, achilles tendinopathy or patellar tendinopathy.
  • Using heat or ice as your only treatment. Neither addresses the underlying cause. They are a bridge to loading and rehab.
  • Skipping the physio. Home ice or heat is helpful, but a persistent injury needs a diagnosis.

⚠️ Seek urgent care if

  • Severe deformity or you cannot bear weight after an injury
  • A pop or crack in a joint with rapid swelling
  • Numbness, pins and needles or weakness after the injury
  • The injury does not improve within a week or is getting worse
