What counts as lower back pain?

Lower back pain is any pain, stiffness or ache in the region between your lower ribs and the top of your buttocks. It affects roughly 4 million Australians at any one time.

Most cases are what we call non specific mechanical low back pain. That means the pain comes from muscles, joints or discs of the lumbar spine, and no single sinister cause is behind it.

Around 90 percent of lower back pain settles within 6 weeks with the right advice, movement and physiotherapy. The rest need a more thorough assessment to rule out red flags.

When is lower back pain a red flag?

Serious lower back pain is uncommon but important to catch early. See your GP or emergency department immediately if you have any of the following.

  • Numbness in the groin or saddle area (possible cauda equina syndrome)
  • Loss of bladder or bowel control
  • Sudden weakness in the legs
  • Back pain after a serious fall, car accident or contact injury
  • Unexplained weight loss, night sweats or fevers alongside back pain
  • A history of cancer with new back pain
  • Severe back pain at night that will not settle with any position

Aside from those red flags, most lower back pain can be assessed and treated by a Sydney physio without any imaging.

Do I need a scan for my back pain?

Short answer: Usually no. Australian and international guidelines specifically recommend against routine imaging for lower back pain in the first 4 to 6 weeks unless red flags are present.

Scans regularly show disc bulges, degeneration or arthritis that do not match the pain and are found in people with no back pain at all. Chasing the scan often leads to fear and unhelpful treatments.

When should you see a physio for lower back pain?

See a physio in Sydney if any of these apply:

  • Your back pain has lasted more than 2 weeks and is not settling
  • Pain is stopping you working, sleeping or exercising
  • Pain radiates down one or both legs (possible sciatica)
  • You have had repeated flare ups of back pain over the last 12 months

How long does lower back pain take to settle?

Short answer: Most acute lower back pain episodes settle within 2 to 6 weeks with the right plan. Chronic lower back pain (lasting more than 12 weeks) takes longer but consistently improves with structured physiotherapy.

  • Days 1 to 3: Keep moving in comfortable ranges. Ice or gentle heat can help. Avoid full bed rest.
  • Week 1 to 2: Symptoms usually start to ease. Book a physio if it is not improving.

Gentle movement including walking is almost always better than full bed rest for lower back pain. Aim for short, frequent walks and avoid staying in one position for too long.
