What Shin Splints Actually Are

Shin splints, properly called medial tibial stress syndrome, is an irritation of the tissues that attach to the inside edge of the shin bone (the tibia). It develops when the load going through your shins consistently exceeds what your bones, muscles and connective tissue are currently conditioned to handle. In short, it is a load problem.

Despite how common it is, shin splints are often poorly managed. Most of the popular fixes (new shoes, more stretching, taking a few days off) only work for as long as you stay off the legs. The moment you go back to your previous training, the pain comes back. That is because none of those strategies address the underlying load mismatch.

Common Symptoms

  • A dull ache along the inside edge of the shin that comes on during running or walking
  • Tenderness when you press along the lower two thirds of the shin bone

What You Should Not Do

Most of the popular shin splints advice misses the actual cause. Avoid these traps:

  • Pushing through sharp pain. Tibial stress reactions can progress to stress fractures if you ignore the warning signs.
  • Buying new shoes as your first move. Footwear can help but rarely solves the underlying load issue on its own.
  • Stretching the calves aggressively when they are flared up. Calf strengthening matters far more than calf stretching for shin splints.
  • Taking 2 weeks off, returning to your previous training volume, then wondering why the pain came straight back.
  • Foam rolling the shins constantly hoping it will release. Foam rolling does not change the underlying load capacity of the tissue.

Do not leave this untreated. Many of our patients tell us they wished they had come in sooner. Shin splints that are ignored can progress to a tibial stress fracture, which is a far more serious injury that sidelines you for months. People who keep training through worsening shin pain often end up with much longer recovery times. Catch it early, address the load issue properly, and most cases settle within 4 to 8 weeks. Push through and you can be sidelined for 12 weeks or more.

When to Book In Immediately

See a physiotherapist or your GP sooner rather than later if you have any of these:

  • Sharp, focal pain in a small specific spot on the shin (a possible stress fracture)
  • Pain that wakes you at night and will not settle
  • Pain that continues at rest, not just with activity
  • Severe swelling, redness or warmth over the shin
  • Pain accompanied by numbness, pins and needles or a tight, swollen feeling in the lower leg during exercise (possible compartment syndrome)

Frequently Asked Questions

How long do shin splints take to heal?

Most cases settle within 4 to 8 weeks with the right plan. Longstanding chronic cases can take longer, and untreated shin splints that have progressed to a stress reaction can take 8 to 12 weeks or more.

Can I keep running with shin splints?

In most cases yes, with modifications. We will guide you on how to adjust your distance, surface, frequency and intensity so you can keep some training going while the shins recover.

Do new running shoes fix shin splints?

Sometimes the shoes contribute, but they are rarely the whole story. The biggest driver is load. New shoes can help as part of a wider plan, but they almost never resolve the issue on their own.

How do I know if it is shin splints or a stress fracture?

A stress fracture usually causes sharper, more focal pain in one specific spot, often hurts at rest, and can wake you at night. Shin splints cause a more diffuse ache along a section of the shin. Your physio will assess and refer for imaging if a stress fracture is suspected.
