What an ACL Tear Actually Is

Soccer player on the ground after a knee injury

The ACL, or anterior cruciate ligament, is one of four major stabilising knee ligaments alongside the MCL, LCL and PCL. It stops your shin bone sliding forward on your thigh bone and controls rotation, which is exactly what you ask of your knee every time you plant and cut in soccer.

An ACL tear (or ACL rupture) happens when the ligament is stretched beyond its capacity, most often during a non contact ACL injury where you land, twist or decelerate awkwardly. Around two thirds of ACL injuries in football are non contact. ACL injuries are graded from a partial tear through to a full ACL rupture, and are often seen alongside meniscus tears or MCL injuries as part of a broader knee ligament injury.

Warning Signs

Common Symptoms of an ACL Tear

  • A loud pop or crack in the knee at the moment of injury
  • Immediate swelling within the first few hours
  • A sensation that the knee gave way or shifted underneath you
  • Pain deep inside the knee, often difficult to point to exactly
  • Difficulty putting full weight through the leg
  • Instability when you change direction or walk down stairs
  • Reduced ability to fully straighten or bend the knee
  • A sense that the knee cannot be trusted in sport, even weeks after the injury

Common Causes of ACL Injuries in Soccer

  • Planting the foot and changing direction sharply, especially on soft or wet grounds
  • Landing awkwardly from a header or aerial challenge
  • Decelerating suddenly with the knee bending inward (dynamic knee valgus)
  • Contact ACL injuries from tackles or collisions while planted
  • Poor movement mechanics under fatigue, especially late in a match
  • Weak hip, glute and hamstring muscles that fail to control the knee
  • Being a female footballer, who has 4 to 8 times higher ACL injury risk than male players
  • Previous ACL reconstruction on the same or opposite knee
  • Missing structured warm ups and injury prevention programs like the FIFA 11+

🏥

Post Surgical Rehab

Structured rehab from day one post op. Range of motion, quads and hamstring strengthening, plyometrics and cutting work.

Return to Soccer

Criteria based return to play testing (hop tests, strength ratios, agility) to confirm your knee is ready for match play.

Avoid These

What You Should Not Do

  • Assume the knee is fine because the swelling has settled. A stable looking knee can still hide a full ACL rupture.
  • Rush back to soccer before your quadriceps, hamstrings and single leg control are properly rebuilt.
  • Skip prehab in the weeks before ACL reconstruction.
  • Rely on a brace instead of proper ACL rehabilitation.
  • Use time alone as your return to sport criteria.
  • Ignore ACL injury prevention once you return to sport.

⚠️ When to seek urgent care

  • Significant knee swelling within an hour of the injury
  • Inability to bear any weight through the injured leg
  • Obvious deformity of the knee
  • A locked knee that cannot be straightened or bent
  • Numbness or pins and needles in the foot or lower leg
FAQs

Frequently Asked Questions

Not every ACL rupture needs surgery. Younger footballers wanting to return to cutting sports usually benefit from ACL reconstruction. Some patients do well with conservative management and structured knee rehab. Your physio and surgeon will guide you.
Typical ACL reconstruction rehab is 9 to 12 months before return to soccer, with strict criteria to be met at each stage. Non surgical ACL rehab timelines vary depending on your goals.
Female footballers have a 4 to 8 times higher ACL injury rate than males due to differences in landing mechanics, hip and knee alignment, hormonal factors and neuromuscular control. Targeted ACL injury prevention programs significantly reduce this risk.
Yes. Structured injury prevention programs like the FIFA 11+ can reduce ACL injury risk in football by up to 50 percent when performed consistently.
