What is an ankle sprain?
An ankle sprain is a stretch or partial tear of one of the ligaments that hold your ankle joint together. It usually happens when you roll onto the outside of the foot.
Most sprains we see are lateral ankle sprains. That means the anterior talofibular ligament (ATFL), and sometimes the calcaneofibular ligament, on the outside of the ankle.
Less common but important to identify are medial ankle sprains (deltoid ligament) and high ankle sprains (syndesmosis injuries). High ankle sprains take longer to recover and often get missed.
How do I know my ankle is sprained?
- Pain and swelling on the outside of the ankle immediately after the rolled ankle
- Bruising that develops within 24 to 48 hours
- Difficulty putting full weight through the foot
- A feeling of instability or that the ankle joint might give way
- Stiffness first thing in the morning that eases with movement
- Repeated ankle sprains on the same side (chronic ankle instability)
- Ongoing ankle pain months or years after the original ankle injury
- Loss of confidence in the ankle on uneven ground, kerbs or stairs
What causes an ankle sprain?
- Landing awkwardly from a jump in basketball, netball, volleyball or soccer
- Stepping off a kerb or uneven surface
- Rapid changes of direction during sport (a classic sports physio Sydney presentation)
- Slipping on a wet floor or stair
- Old ankle injuries that were never fully rehabilitated leading to chronic ankle instability
- Weak hip and glute muscles altering the way you land and absorb force
- Poor ankle proprioception (your sense of where the ankle is in space)
- Returning to sport too soon before ankle rehabilitation is complete

6 mistakes that slow ankle sprain recovery
- Rest the ankle completely until pain disappears then jump straight back into sport. That is the fastest way to reinjure the ankle.
- Ignore the balance and ankle proprioception work. Strength alone will not prevent another rolled ankle.
- Wear an ankle brace forever without doing the underlying ankle strengthening. Braces are a tool, not a fix.
- Assume the absence of pain means full recovery. Ankle strength and proprioception take longer to rebuild than pain takes to fade.
- Skip imaging when the ankle is severely swollen, badly bruised or cannot bear weight. Some ankle injuries hide fractures.
- Ignore repeated ankle sprains. Each one stretches the ligaments further and drives chronic ankle instability.

- Unable to bear any weight on the foot 4 steps after the ankle injury
- Pain directly on the bony bumps on either side of the ankle joint (Ottawa Ankle Rules positive)
- Severe swelling or bruising that develops within an hour

- Numbness or pins and needles in the foot or toes
