Rotator Cuff vs Frozen Shoulder explained
Rotator Cuff or Frozen Shoulder: What Is the Difference?
Your rotator cuff is a group of four muscles and tendons that steady and lift your arm. When they get overloaded or torn, lifting hurts and feels weak. A frozen shoulder is different: the joint capsule itself tightens and thickens, so the shoulder slowly loses movement in every direction. The good news: both get better with the right physiotherapy.

- Waking at night when you roll onto that side
- Lifting your arm out to the side pinches or catches

Rotator Cuff vs Frozen Shoulder Side by Side
They can feel alike at first, but they behave very differently. Here is how a rotator cuff problem and a frozen shoulder usually compare.
Where it hurts
Outside of the upper arm, worse lifting out to the side or overhead
Deep in the shoulder, all over, sometimes spreading down the upper arm
What it feels like
A painful catch or pinch through the middle of a lift, often with weakness
A deep ache plus stiffness, like the shoulder is stuck or locked
Pain pattern
Comes and goes with load, worse after gym, overhead work or lying on it
Builds slowly over weeks, very painful early, then stiffness takes over
Can you lift your arm
Usually yes with help, even if lifting it yourself hurts or feels weak
Not far, even when someone else lifts it, especially out to the side or behind you
Common causes
Overload from gym, overhead work or sport, wear with age, or a fall
Often no clear cause, more common from 40 to 60 and with diabetes
Typical recovery
Often much better in 6 to 12 weeks, with strength building for up to 6 months
Moves through painful, stiff and thawing stages, often over 1 to 3 years
Best first step
A physio assessment and a progressive rotator cuff strength program
A physio assessment, pain relief strategies and gentle, regular movement within comfort
AIt is more likely your rotator cuff if lifting hurts and feels weak, but the arm still moves well when someone helps.
BIt is more likely a frozen shoulder if it is stiff in every direction, even when someone else moves your arm for you.
A physio assessment sorts it out, with same day appointments at Bexley North and Marrickville and a clear plan from your very first visit.

Rotator Cuff, Frozen Shoulder or Something Else?
Shoulder pain is not one thing. Your physiotherapist works out which of these is driving yours, because each one needs a different plan.
01Rotator Cuff Tendinopathy
Feels like: ache down the outside of your arm, worse lifting out to the side or lying on it.
The cuff tendons get overloaded, often after more gym, overhead work or sport. The shoulder still moves, and progressive strength work is the most proven fix.
Read about rotator cuff physio →02Rotator Cuff Tear
Feels like: weakness lifting your arm, sometimes after a fall or a heavy pull.
Part or all of a tendon is torn. Many do well with strength work, and we arrange a scan and surgical opinion quickly when needed.
03Frozen Shoulder
Feels like: deep ache, then stiffness, unable to reach behind your back or out to the side.
The joint capsule tightens and thickens, most often between 40 and 60. It moves through painful, stiff and thawing stages, and physio helps at each one.
Read about shoulder pain physio →04When Both Overlap
Feels like: started as a sore rotator cuff, now stiff in every direction.
A painful shoulder that is kept still can stiffen, and early frozen shoulder can look a lot like cuff pain. Testing movement and strength tells us what to treat first.
05Shoulder Arthritis
Feels like: stiff, grinding and achy, most often over 60.
Worn joint surfaces can stiffen a shoulder much like a frozen shoulder. An X ray tells them apart, and strength and movement work help both.
06Pain From Your Neck
Feels like: ache over the shoulder blade or down the arm, sometimes with tingling.
An irritated joint or nerve in the neck can refer pain into the shoulder. We check your neck so the right area gets treated.

How Long Does Each Take to Get Better?
Rotator cuff pain and frozen shoulder recover on very different timelines. This is what it usually looks like with a physiotherapist guiding you.
- First 2 to 6 weeks
Settle the Pain
Most rotator cuff pain eases as we adjust your load. In early frozen shoulder, we focus on easing pain and keeping gentle movement.
- Weeks 6 to 12
Build Strength or Range
Rotator cuff strength builds and most people lift and sleep much better. A frozen shoulder may be at its stiffest around now.
- Months 3 to 12 and beyond
Strong and Moving
Cuff strength keeps building for up to 6 months. A frozen shoulder thaws gradually, often over 1 to 3 years in total.
Tears, surgery and stubborn frozen shoulders can take longer, and some people benefit from a GP or specialist opinion about injections. Your physio will give you your own timeline, or call (02) 8021 0969 to ask.

?Quick answers
Rotator Cuff or Frozen Shoulder Quick Answers
Fast answers to the questions people search most about a sore, stiff shoulder.
How can I tell frozen shoulder from a rotator cuff problem?
Check the stiffness
With frozen shoulder the arm will not move far even when someone else lifts it. With a rotator cuff problem lifting hurts and feels weak, but the arm usually still moves when helped.
Can you lift your arm with frozen shoulder?
Only partly
Usually only part of the way. Frozen shoulder limits movement in every direction, especially reaching out to the side and behind your back, even when someone helps lift the arm.
Can a rotator cuff injury cause frozen shoulder?
Sometimes
It can. A painful shoulder that stops moving can stiffen, and some people develop frozen shoulder after an injury or surgery. Gentle, guided movement helps lower that risk.
Does frozen shoulder go away on its own?
Usually, slowly
Most frozen shoulders improve over time, often across 1 to 3 years. Physio eases the pain and helps you keep and regain movement through each stage, so daily life is easier along the way.
Is rotator cuff pain or frozen shoulder worse at night?
Both can be
Both often ache at night. Rotator cuff pain usually flares when you lie on that side, while early frozen shoulder can ache constantly. Hugging a pillow or resting the arm on one helps.
Do I need a scan to tell them apart?
Usually not
Most of the time a hands on assessment of strength and movement tells them apart. An ultrasound can check the rotator cuff, and an X ray can rule out arthritis if needed.
When to See a Physio, a Doctor, or Get a Scan
Most shoulder pain is not dangerous, but a few warning signs need urgent care. Here is how your physiotherapist decides what you need.
✓Book a Physio If
These are the most common reasons people see us.
- Lifting your arm out to the side or overhead hurts
- Your shoulder is slowly getting stiffer
- Pain wakes you at night or stops you lying on that side
- It stops you working, training or dressing easily
!See a GP or Hospital Now If
These red flags need urgent medical care.
- Shoulder or arm pain with chest pain, shortness of breath, sweating or jaw pain (call 000)
- Obvious deformity or the shoulder has popped out and stayed out
- Hot, red, swollen shoulder with fever
- Sudden numbness or weakness down the arm
- Unexplained weight loss, a new lump or a history of cancer
?Do You Need a Scan?
Most people do not need one straight away.
- An ultrasound or MRI can check for a tear if you cannot lift your arm after a fall or pull
- An X ray helps rule out arthritis or calcium build up when a shoulder is very stiff
- If you need one, we liaise with your GP or specialist for you

The big clue is stiffness. With frozen shoulder the arm will not move far even when someone else lifts it. With a rotator cuff problem lifting hurts and feels weak, but the arm usually still moves when helped.
It can happen. A painful shoulder that you stop moving can stiffen, and some people develop frozen shoulder after an injury or surgery. Keeping it gently moving with guidance helps lower the risk.
It is most common between 40 and 60, more often in women, and in people with diabetes or thyroid conditions. It often starts for no clear reason.
It usually moves through painful, stiff and thawing stages over 1 to 3 years, though many people improve sooner. Physio eases the pain and keeps you moving at each stage.
Not always. A hands on assessment usually tells them apart. An ultrasound can show the rotator cuff tendons, and an X ray helps rule out arthritis in a stiff shoulder. If you need one, we arrange it through your GP.
No. You can book straight in at Bexley North or Marrickville, with same day appointments most days. You only need a referral for a Medicare care plan, DVA, Workers Compensation or CTP.
Your first visit is a full 60 minutes, one on one. That gives us time to test your strength and movement, find the real cause and build a clear, structured plan.
Yes. We have HICAPS at both clinics, so you can claim on the spot. We also see Medicare care plan, DVA, Workers Compensation and CTP patients.
