Why Shoulder Pain Happens
The shoulder is the most mobile joint in the body. That mobility comes from a small group of stabilising muscles called the rotator cuff, plus the muscles around your shoulder blade, neck and upper back. When even one of those links is overloaded, under strong, or moving poorly, the whole system starts to grumble.
Most shoulder pain we see in clinic is not from a single tear or a dramatic injury. It builds quietly from repeated overhead reaching, poor desk posture, heavy gym sessions without a balanced program, or a sudden spike in load like painting a ceiling or starting a new sport. The good news is that strong evidence shows progressive loading and targeted physiotherapy outperforms cortisone and surgery for the vast majority of rotator cuff problems.
Common Symptoms
- A deep, dull ache in the front or outside of the shoulder
- Sharp pain reaching overhead, into a seatbelt, or behind your back
- Pain that wakes you at night when you roll onto that side
- Weakness lifting a kettle, a child, or a barbell off the rack
- Clicking, catching or a feeling the shoulder is not tracking properly
- Pain referring down the outside of the arm toward the elbow
Common Causes
- Rotator cuff tendinopathy from repetitive overhead load
- Subacromial impingement and bursitis
- Rotator cuff tears, partial or full thickness
- Frozen shoulder (adhesive capsulitis) in the 40 to 60 age range
- Shoulder instability from gym, surfing, swimming or contact sport
- AC joint sprains from a fall onto the point of the shoulder
- Referred pain from a stiff neck or thoracic spine

How We Treat Shoulder Pain at PHYZFIT
Our physios take a structured, evidence based approach. We do not just hand you a sheet of band exercises. We work out exactly which tissue is irritated, why your shoulder ended up there, and then we load it in the right direction at the right intensity until it is bulletproof again.
Accurate Diagnosis
We take a detailed history then run a series of clinical tests for the rotator cuff, the labrum, the AC joint, the cervical spine and the scapular muscles. If we suspect a structural tear or a referral pattern we are not happy with, we coordinate imaging with your GP or a sports physician. You leave the first session knowing exactly what is going on.
Rotator Cuff Strengthening
The research is now very clear: progressive loading of the rotator cuff is the most effective intervention for shoulder pain. We start with isometric holds to settle the irritated tendon, progress through controlled rotator cuff work, then build into pressing, pulling and overhead patterns at full load. Most people feel a meaningful shift inside the first 3 to 4 sessions.
Scapular and Postural Retraining
Your shoulder blade is the foundation the whole arm moves on. If it is sitting forward, tipping, or not rotating properly under load, the rotator cuff has no chance. We coach scapular control, mid back mobility, and the postural strength you need so the cuff is not the only structure doing the work.
Hands On Therapy
We use targeted soft tissue release through the upper traps, pec minor, posterior cuff and lats, joint mobilisations through the thoracic spine and shoulder, and dry needling where it helps to settle irritable tissue. This is never the whole treatment. It is the short cut that lets you load properly afterwards.

What You Should Not Do
- Push through sharp pain at the gym. Tendons respond to graded load, not punishment.
- Rest for weeks hoping it settles on its own. Cuff and bursa issues often get stiffer and weaker the longer you wait.
- Rely on rolling and stretching alone. Most shoulder pain is a strength and control problem, not a tightness problem.
- Jump straight to cortisone before a proper rehab trial. The evidence shows short term relief but worse outcomes at 6 and 12 months.
- Ignore neck stiffness or upper back posture. They drive a surprising amount of shoulder pain.
Do not leave this untreated. Shoulders that grumble for months often progress to thicker tendon changes, partial tears, capsule stiffness, and altered movement patterns that take much longer to unwind. Early loading and rehab almost always avoids surgery.

Frequently Asked Questions
How long does it take to fix a shoulder injury?
Most rotator cuff and impingement cases settle meaningfully within 4 to 6 weeks of structured rehab, with full strength typically rebuilt over 8 to 12 weeks. Frozen shoulder, post surgical cases and longer standing issues take longer, and we map that out for you on day one.
Do I need a scan first?
Usually no. A skilled physio can diagnose the vast majority of shoulder problems clinically. Imaging is reserved for cases where we suspect a significant tear, where pain is not responding as expected, or where surgical opinion is on the table.
Will I need surgery?
The vast majority of shoulder pain, including most rotator cuff tears, does very well with physio. Surgery becomes a conversation only when a large traumatic tear, recurrent instability or a structural problem is not responding to a good loading program.
Can I keep training while I rehab?
Almost always yes. We modify the load, change the angle, and pick exercises that strengthen the shoulder without flaring it. Stopping the gym entirely is usually the wrong answer.
