What actually is chronic back pain?

Chronic back pain is back pain lasting longer than 3 months. It affects around 1 in 6 Australians at any given time and is one of the top causes of disability, missed work days and reduced quality of life. It is different to acute back pain in one critical way: the pain is no longer a reliable signal that tissue is being damaged.

Modern pain science shows that chronic pain is driven by a combination of factors:

  • Nervous system sensitisation: the pain system becomes more sensitive over time and starts producing pain in response to normally safe movements
  • Deconditioning: avoiding movement leads to weaker muscles, stiffer joints, worse posture and less capacity to handle load
  • Fear avoidance: the belief that movement will damage you further leads to less movement, which drives more sensitivity and more pain
  • Sleep and stress: poor sleep and high stress lower the pain threshold significantly
  • Beliefs about the pain: "my disc is crumbling", "my spine is unstable", "I need to protect my back" all amplify the pain experience
  • Occasional structural contributors like a disc bulge, facet joint arthritis or spondylolisthesis, but these are rarely the whole story

This is important because it changes what treatment actually works. Chronic back pain does not respond well to "protection" strategies. It responds to a structured, graded return to movement, load and life.

Pain education and belief reset

We walk you through modern pain science in plain English. How chronic pain works, why hurt does not always equal harm, what your scan actually means (and does not mean), and why movement is now medicine. This step alone often reduces pain significantly by taking fear out of the picture.

Hands on treatment and movement retraining

What can I do at home right now?

Whether or not you are booked in yet, these habits move the needle on chronic back pain from day one:

  • Walk daily. 20 to 30 minutes, most days, at a comfortable pace
  • Change position every 20 to 30 minutes when sitting
  • Sleep 7 to 9 hours and address any sleep disruption
  • Reduce stress load where you can (stress lowers your pain threshold significantly)
  • Do gentle mobility work daily. Cat cow, knee rocks, glute bridges
  • Track your triggers and wins in a note on your phone
  • Stop searching worst case scenarios online at 2am — it makes the pain worse

Common myths about chronic back pain

  • Myth: "My spine is crumbling." Scans in pain free people over 40 show almost identical "wear and tear" findings as in people with pain. The scan does not equal the pain source in most chronic cases.
  • Myth: "I need to protect my back." Over protecting is a major driver of chronic pain. The evidence shows the opposite of protection is what helps.
  • Myth: "Bed rest will help." Bed rest weakens the muscles that stabilise the spine and increases pain sensitivity within days.
  • Myth: "I need another scan." Rarely helpful in chronic back pain. Repeat scans usually show the same "findings" and rarely change treatment.
  • Myth: "Surgery will fix me." Surgery has a role for very specific presentations (severe stenosis, cauda equina, progressive weakness, unstable spondylolisthesis) but is rarely helpful for non specific chronic back pain.
  • Myth: "It is just in my head." Chronic pain is 100 percent real. It is a real nervous system output. It also responds to nervous system inputs like education, movement, sleep and stress management.

⚠ Seek prompt medical review if you have

  • Loss of bladder or bowel control, or new saddle numbness (possible cauda equina, go to Emergency)
  • Progressive weakness in both legs
  • Unexplained fever, weight loss or night sweats with back pain
  • History of cancer with new persistent back pain
  • Severe unrelenting pain not settling in any position
  • Back pain after significant trauma or a fall

How long will it take to see meaningful improvement?

Realistic expectations based on the evidence:

  • 2 to 4 weeks: pain education and belief reset often produces the first meaningful drop in pain intensity
  • 4 to 8 weeks: movement tolerance improves, flare ups become less frequent and less severe
  • 8 to 16 weeks: around 80 percent of patients report meaningful reductions in pain and function
  • 3 to 6 months: return to full life activities including gym, sport, heavy work
  • 6 to 12 months: full self management with occasional physio check ins

Frequently asked questions about chronic back pain

Any back pain lasting longer than 3 months is classified as chronic. It does not mean permanent. It means the pain has moved past the normal tissue healing window and needs a different treatment approach than acute back pain.

Usually not. Repeat scans in chronic back pain rarely change the treatment plan. Imaging is considered if there are new red flag features, sudden change in symptoms, or when a surgical opinion is being planned.

Structured graded exercise is one of the strongest evidence based treatments for chronic back pain. Starting at the right intensity and progressing gradually is the key. That is what your physio guides.

Most chronic back pain patients need 8 to 14 sessions across 3 to 4 months to move through the full pathway, then graduate to monthly or as needed maintenance. This is very different to weekly forever.

Short term use of paracetamol or anti inflammatories under GP guidance can help flare ups. Long term opioids are generally not recommended for chronic back pain because they lose effectiveness and carry significant side effects.
