Why Achilles Tendinopathy Happens

The Achilles tendon is the thickest and strongest tendon in your body, connecting the calf muscles (gastrocnemius and soleus) to your heel bone. It is designed to absorb and produce enormous forces during running, jumping and everyday walking. But when the load placed on it exceeds its capacity to recover, the tendon tissue begins to degenerate. This is tendinopathy, and it is different from tendinitis. There is no significant inflammation involved. The tendon structure itself has changed, which is why anti inflammatory medication and rest alone almost never fix it.

Accurate Diagnosis and Load Assessment

What You Should Not Do

  • Do not push through significant pain and continue training at full intensity
  • Do not rest completely for weeks as the tendon needs progressive load to heal
  • Do not aggressively stretch the Achilles in the early stages as this increases compressive load on the tendon
  • Do not take anti inflammatories long term expecting them to fix the underlying tendon issue
  • Do not ignore it and hope it resolves without addressing your training load and contributing factors

Frequently Asked Questions

Is it safe to keep running with Achilles tendinopathy?

It depends on the severity and stage of your tendinopathy. In many cases a modified training load can be maintained throughout rehabilitation. Complete rest is rarely the answer. Your physio at PHYZFIT will establish exactly how much you can do and build a graded return to running plan within your program.

How long does Achilles tendinopathy take to heal?

Most cases of Achilles tendinopathy see significant improvement within 6 to 12 weeks of consistent physiotherapy. Full return to sport can take 3 to 6 months depending on the severity of the tendon changes and how quickly the calf strengthens. Tendons respond slowly but they do respond with the right stimulus.

Will I need surgery for Achilles tendinopathy?

The vast majority of cases do not require surgery. Physiotherapy rehabilitation with progressive loading has very strong evidence for full recovery. Surgery is considered only for cases that have failed at least 3 to 6 months of high quality conservative management. Starting treatment early significantly reduces the chance of ever needing surgical intervention.

Do I need a referral to see a physio at PHYZFIT?
